This form is for potential beneficiaries (or their representative) of a grant from the MY Foundation to make an application for nomination. If nominated the applicant will be required to provide additional information and the full submission will be reviewed by the Foundation.

Unfortunately we only have limited funds and so can only support a limited number of grants each year.
